[1] Take a new file of 500 pixels 500 pixels of resolution 72 dpi in the RGB mode.

[2] Fill the new document with color black.

[3] Take the Text Tool and create the text.

[4] Then go to Filter>Texturizer>Stained Glass
use the following settings.

[5] Your image should be look like this.

[6] Now go to Select>Color Range and click
on the white of the stained glass to select it.

[7] Your image should be look like this.

[8] Now go to Channels Pallete create a new channel
and fill the selection with white.

[9] Your should be look like this.

[10] And Press Cntl+D to deselect.

[11] And then go to Filter>Blur>Gaussian Blur
apply the following settings.

[12] Now go to Image>Adjustments>Levels
use these settings.

[13] Your image should be look like this.

[14] Now go to Select>Load Selection.
In the channel field select Alpha1 this will
load the select from the channels pallete.

[15] Now create the a new layer.

[16] And fill this selection with
grey then press Cntl+D deselect.

[17] Now select the Drop Shadow from the
Layer style and use the following settings.

[18] Now select the Inner Shadow from the
Layer style and use the following settings.

[19] Now select the Bevel and Emboss from
the Layer style and use the following settings.

[20] Your image should be look like this.

[21] Now change your background
color and here is your final image.
