using photoshop.

[1]-First of all open a new document size 400x300 pixel
96 resolution, RGBcolor mode

[2]- Create a new Layer1 from the Layer window
and now create a new shape with Rectangular
marquee tool now fill the gray color with Alt+Backspace

[3]-Now go to Select>Modify>Contract
selelection and use the settings below

[4]- Press the 'Delete' key on your
keyboard . As shown below

[5]-Go to Select>Modify>Border Selection
now use the settings below

[6]-Create new layer2 from the layer window
Now fill the Black color with Alt+Backspace

[7]- Double click on this Layer2 and select
the Bevel and Emboss from the Layer style
now use the settings below

[8]- Here Double click on this layer1
and select the Bevel and Emboss from
the Layer style now use the settings below

[9]- Press the "D" key on your keyboard
and Type any words with TEXT TOOL .

[10]- Double click on this Text layer and select
the Bevel and Emboss and use the settings below
do not click ok

[11]- Now select the Drop Shadow and
use the settings below . now click ok

[12]- Your image has been completed . As shown below
